Why it happens
Teams buy because it is faster, then live with constant exceptions. Or they build because they can, then discover they cannot operate, train, or change the system without heroes.
Leadership implication
Ask not only “can we build it?” but “can we operate it?” Ownership, training, and a change process are part of the decision - not follow-up work.
